张继辉:抑郁症的患病率(lv)大約(yue)是(shi)2%-3%,大部(bu)分(fen)抑郁症患者症状還(hai)是很(hen)明(ming)顯(xian)的,比(bi)如情(qing)緒(xu)低(di)落(luo)、憂(you)心忡(chong)忡、行(xing)動(dong)緩(huan)慢(man)、唉(ai)聲(sheng)嘆(tan)氣(qi)等(deng)。但(dan)是,我们要(yao)警(jing)惕(ti)“微(wei)笑(xiao)型(xing)抑郁症”和“隱(yin)匿(ni)性(xing)抑郁症”,这些患者往(wang)往內(nei)心很痛(tong)苦(ku),但是從(cong)外(wai)觀(guan)来看並(bing)不容(rong)易(yi)被察(cha)覺(jiao)到(dao)。

张继辉:抑郁症有两个核心症状:一是情绪低落,總(zong)是郁郁寡(gua)歡(huan);二(er)是对周(zhou)圍(wei)的事(shi)物(wu),甚(shen)至(zhi)对过往喜(xi)欢做(zuo)的事情都(dou)失(shi)去(qu)興(xing)趣(qu),出现快(kuai)感缺(que)失。除(chu)此(ci)之(zhi)外,患者还會(hui)出现食(shi)欲(yu)差(cha)、體(ti)重(zhong)下(xia)降(jiang)、无法(fa)正(zheng)常(chang)履(lv)行工(gong)作(zuo)和家庭(ting)職(zhi)責(ze)、維(wei)持(chi)良(liang)好(hao)的社(she)交(jiao)關(guan)系(xi)等。

若(ruo)出现上述(shu)两个核心症状,就可(ke)以使(shi)用(yong)PHQ-9(抑郁症篩(shai)查(zha)量(liang)表(biao))測(ce)試(shi)進(jin)行早(zao)期(qi)筛查,初(chu)步(bu)判(pan)斷(duan)是否有抑郁倾向。需(xu)要注意的是,以上方法僅(jin)能(neng)作为抑郁症輔(fu)助判断手段(duan),確(que)診(zhen)抑郁症必(bi)須(xu)要经过专業(ye)医生臨(lin)床(chuang)诊断。

此外,内分泌(mi)紊(wen)亂(luan)、器(qi)質(zhi)性疾(ji)病等可能也(ye)会造(zao)成(cheng)抑郁情绪,临床上也需要仔(zai)細(xi)甄别。

张继辉:一般(ban)情況(kuang)是情绪低落症状持续两周以上,且(qie)超(chao)过一半(ban)时间都感到悶(men)闷不樂(le)或(huo)明显情绪低落。抑郁症大多是波(bo)动性病程(cheng),即(ji)会反复发作,往往会在少(shao)則(ze)两周、幾(ji)个月,多则两三(san)年中反复发作。

南方+:如果(guo)使用网上的抑郁症筛查量表进行测试後(hou),发现自(zi)己(ji)有抑郁倾向,怎(zen)麽(me)辦(ban)?

张继辉:首(shou)先(xian),不要惊慌(huang)。抑郁症筛查量表的测试結(jie)果并不能用来确诊抑郁症。比如PHQ-9测试显示(shi)是轻度(du)抑郁,如果沒(mei)有影(ying)響(xiang)社会功(gong)能,并不需要太(tai)过惊慌。此时,可以做些放(fang)松(song)訓(xun)練(lian)、多運(yun)动,缓解(jie)自己的壓(ya)力(li)。

如果测试结果显示中度或以上程度抑郁,时间又(you)持续两周以上,明显影响到自己的正常生活(huo)和工作,那(na)么就要及(ji)时到医院精(jing)神(shen)科就诊,通(tong)过专业檢(jian)查和評(ping)估(gu),才能诊断是否患有抑郁症。

如果患有抑郁症也不用惊慌,60%-70%抑郁症患者吃(chi)抗(kang)抑郁藥(yao)就可以得到良好的治(zhi)療(liao)效(xiao)果。除了药物治疗,还有行为疗法、中医調(tiao)理(li)等。

南方+:如何消除抑郁症病恥(chi)感?

张继辉:第(di)一,要加(jia)強(qiang)宣(xuan)傳(chuan)教(jiao)育(yu),抑郁症很常見(jian),患上这个病并不是一件(jian)羞(xiu)耻的事情;第二,抑郁症是可防(fang)可治的,不用太过擔(dan)心;第三,病耻感不利(li)于病情恢(hui)复,大多数抑郁症患者对外界(jie)的破(po)壞(huai)力極(ji)低,我们要科学看待(dai)抑郁症。

南方+:怎么區(qu)分抑郁和焦(jiao)慮(lv)?

张继辉:抑郁的两个核心症状就是情绪低落和快感缺失,是对过去发生的事情感到懊(ao)悔(hui)、煩(fan)躁(zao);而(er)焦虑多是緊(jin)张不安(an),担心未(wei)来会发生不好的事情。

但是,抑郁和焦虑往往还会一起出现,有點(dian)像(xiang)“兄(xiong)弟(di)”,50%-60%焦虑症患者同(tong)时会患有抑郁症,反之也一樣(yang)。

南方+:如果我们遇(yu)到了身边患有抑郁症的朋(peng)友,如何帮助他们走(zou)出抑郁的泥(ni)潭(tan)?

张继辉:抑郁症是一種(zhong)疾病,而不是患者本(ben)人有意控(kong)制(zhi)和選(xuan)擇(ze)的结果。我们往往容易將(jiang)抑郁症單(dan)純(chun)看作是心情不好而已,因(yin)此我们会肆(si)意去否定(ding)他们当前的感受(shou),甚至无端(duan)地(di)指(zhi)责。

我们要学会倾聽(ting),不要总是談(tan)論(lun)自己,用自己的经歷(li)去否認(ren)和懷(huai)疑(yi)他当前的感受;“听”是要尊(zun)重对方,不要打(da)断他的对話(hua),问问自己“是否我们真(zhen)的理解他的问题了”,不要急(ji)于去评價(jia)对方的观点,不要急切(qie)表達(da)建(jian)議(yi)。
